Evergreen Canine LLC reserves the right to take pictures, video and record puppy visiting our home, to be used for any media and YouTube (public).
Though Evergreen Canine LLC strives to provide a safe and clean environment for any visiting or resident dogs, accidents & illness can happen at anytime and Evergreen Canine LLC shall be held blameless. Owner of the dog described above promises that the dog has come from a clean and healthy environment and has had his/her veterinary health check verifying that the dog is currently healthy and does not have any known disease or issues. Owner is responsible to make sure that dog's vaccines and dewormings are up to date according to his/her age before bringing dog to our home. If dog arrives and develops any loose stools that cannot be quickly explained, Evergreen Canine LLC may charge $40/ in house test to determine the cause of the loose stool. This is required to lessen the risks of potential contamination. If the cause of loose stool requires medication or cannot be identified by our tests, the owner will be responsible for any and all vet bills incurred and for any further testing & medication needed to resolve the loose stool. Should the dog suffer from any accident or illness of any kind that requires veterinary care, the owner understands that the owner of the dog is 100% responsible for any and all vet charges. Evergreen Canine LLC will communicate with the owner in regards to which vet will be used for the care needed. Evergreen Canine LLC strongly suggests owner purchase health insurance for their dog prior to their boarding stay to lessen potential costs that are unknown should accident or illness occur.
Evergreen Canine LLC strives to keep a safe and sanitary environment and follows strict protocols to clean and sanitize all dogs areas daily.
This is considered a legal binding document. If at any point legal action is needed, all claims and or court filings will be held in the state of Utah. Evergreen Canine LLC reserves the right to refuse to provide services to any customer if deemed necessary.
